Due to the hectic holiday season, the deadline for the Olympia Shoutout Ads has been extended to Tuesday, January 7th.

Please return this form along with payment to the OGBC box.
Checks made payable to OGBC.
Please email photos to: olympiashoutoutad@yahoo.com and reference the gymnast's name.
All ads need to be turned in by Tuesday, January 7th, 2020.
